
ACCIDENT REPORTING, RECORDING AND INVESTIGATION MANAGER/SUPERVISOR RESPONSIBILITIES

The following definitions apply to the terms and conditions set out below that govern the contract of services between you and us.

By entering the Builders worksite and engaging in work you agree to adhere to this policy.

Policy
The Builder will ensure that all accidents are reported, recorded and investigated.
Procedure
- The Manager / Supervisor of the area where the accident occurred will undertake an investigation within 24 hours of the occurrence.
- The Manager /Supervisor of the area will implement temporary controls to prevent any further injuries to employees.
- The Manager/Supervisor will review the equipment, operations and process to gain an understanding of the accident situation.
- The Manager/Supervisor will identify and interview each witness and any other person who might provide clues to the accident's causes.
- The Manager/Supervisor will investigate causal conditions and unsafe acts, make conclusions based on existing facts only.
- The Manager/Supervisor will complete the accident investigation report.
- The Manager/Supervisor will provide recommendations for corrective actions.
- The Manager/Supervisor will indicate the need for additional or remedial safety training.
- The Manager/Supervisor will ensure 1-8 above is documented.
